摘要: 深層腦刺激手術為減緩大腦部正常分泌多巴胺所導致的症狀,但觀看整個手術過程中無法客觀判斷目前推進的探針位置到底在何處,僅能利用醫療團隊經驗及最後與病患的互動式檢測,因此希望利用磁場傳遞不受人體影響概念在外部產生磁場,於探針尖端接收形成感應電壓進行定位,並且結合多年成熟的有限元素分析和類神經網路進行校正,使手術過程可以直接使用校正過後的參數進行定位,假使因環境干擾形成的雜訊便以強度定位法進行修正,最後以二維及三維影像作表達方式,可以直觀判斷出手術進行狀況
In this study, a real-time positioning system for deep brain stimulation (DBS) lead probe is developed. During DBS surgeries, surgeons insert a DBS probe to identify the target of the DBS stimulation. This usually involves two to four insertions since the pre-surgeries computed tomography (CT) images and Magnetic Resonance Images (MRI) do not provide surgeons enough image resolution. To overcome this limitation, surgeons use the micro-electrode recording (MER) technique to identify the target and its depth during surgeries. Unfortunately, the use of MER signal to estimate the position of the DBS probe or electrode is based on experience of the surgeons. Once the position of the target is found, a permanent DBS electrode is inserted to the target at the depth found by using the MER technique. After surgeries, surgeons confirm the position of the DBS electrodes using X-ray or computed tomography. If there is an error, the DBS electrode has to be explanted and re-implanted. Consequently, there is a need to assess the position of the DBS probe or electrode in real time during surgeries instead of after the surgeries.
