摘要:   醫院的加護病房是一個多人、多作業和多資訊的大型複雜系統,醫師每日例行的晨會和巡房作業尤其需要大量且最新的病患資訊,醫師透過收集和整合這些資訊,作為診斷和決定治療計畫的基礎。目前的醫院資訊系統分散,造成醫師的高工作負荷和高認知負荷,產生的人為疏失也較多。因此,發展一個能輔助醫師快速地整合資訊並作出正確的決策的資訊系統,為目前醫療系統的重要工作。   本研究將醫師晨會和巡房作業分別進行作業分析,了解詳細的作業內容、人與設備、人與人和人與環境的互動,進而找出晨會和巡房時,醫師和其他成員需要的資訊,和資訊擷取到轉移的過程。之後使用認知作業分析,確認醫師進行決策時的推理行為。   了解人員作業的資訊需求後,根據醫師的認知行為,發展資訊系統的功能和架構,最後設計介面雛型,具體呈現出本研究所提出的資訊系統功能如何輔助醫師的協同診斷作業,期望能以此資訊系統改善醫師的診斷品質,降低醫師的負荷,防範可避免的醫療錯誤,保障病患的安全。
Intensive Care Unit (ICU) is a large and complex system in hospitals, with many members processing information to complete lots of tasks. Physicians in ICU perform morning meeting and interdisciplinary round every day, the two tasks especially demand a large amount of newest patient information. With collecting and integrating these information, to diagnosis and prepared for treatment plan. Currently, the information systems in the ICU are separated. As a result, physicians are under high physical workload and mental load, also causing human errors. To address this problem, developing a system support doctors quickly collect and integrate information is an important issue. Through analyzing morning meeting and round task, identify detail task, interaction between users, devices, members and environment. What’s more, to produce the information flows. At the last stage, use cognitive task analysis to analyze user behaviors in problem-solving. After identifying the information requirements and the functions of the systems, construct the prototype, which can show how the new system supporting medical problem-solving in ICU. This research expects to improve diagnosis quality, reduce workload and errors, maintaining patient safety.
