摘要: 核电厂人员作业绩效对于核能安全影响甚大。许多重大核能事故与事件皆与人员作业有关。虽然人为疏 失是事件、事故之近因,但是事故肇因分析的结果发现其背后大多因为系统设计的不良所引起,而这些系统 设计的问题又大多渊源于人员与作业间之不协调。因此,如果要提升核能安全,首要之务乃是探讨核能运转 人因失误问题,确认其肇因,发展解决方案,回馈于核能安全之设计。 本计画之主要目的在于建立核电厂之运转作业人因失误的资料库,分析其肇因,依肇因归类,以对TRR- II的人因工程设计提供设计回馈。其研究步骤包括:(1)从事故、事件档案中,确认与人员作业绩效问题有 关之事故、事件;(2)重建事件经过;(3)进行肇因分析;(4)将肇因归类;(5)建立人员作业绩效问题资料库 与查核表。研究结果可应用于核电厂人员作业绩效改善及TRR-II主控室人因设计独立验证与确认之依据(In- dependent Verification and Validation)。
Human performance has a great impact on nuclear safety. Studies have found that most of nuclear accidents and incidents were related to human errors. Although human error is one of the proximal cause for those accidents, it nevertheless is caused by human-tasks mismatches. To enhance nuclear safety, it is important that human performance problems be identified, causes of those errors be analyzed, and finally feedback to the design of new systems. The purpose of this study is to establish the nuclear operational human error database. The research method consists of the following steps: (1)to identify human performance related accidents/incidents, (2)to analyze root causes of these human performance problems, (3)to establish human performance problem tracking system, and (4)feedback to the human-system inter- face design of TRR-II.
